• The number of members your Lodge has of April 1, 2008.
  This number remains the same for the entire year.

• The amount of Piggy Bank money your Lodge has in the bank. DO NOT report what has been sent to Fresno.

• The number of new 52 Club members your Lodge submitted this month.

• The number of Century Club members your Lodge submitted this month.

• The number of Form 11's your Lodge has sent to Fresno this month.

• The number of Millennium Club members your Lodge submitted this month.

The Lodge Chairman should contact the District Chairman whenever a certificate is received from Fresno. The district Chairman should be invited to attend the presentation ceremony.

Please send a copy of the invitation to the State Chairman and Vice Chairman (North), especially for any Millennium Certificates and Pins.

This report should be submitted to your District Chairman no later than the 3rd of every month to report the figures for the previous month.
